Position Summary: Our Client is seeking an Registered Nurse (RN) in Long-Term Care provides comprehensive nursing care to residents in skilled nursing and assisted living facilities. This role involves assessing patient needs, developing and implementing individualized care plans, and coordinating with interdisciplinary healthcare teams to ensure optimal patient outcomes. The RN ensures adherence to state and federal regulations while promoting a safe, compassionate, and supportive environment for residents and their families. Key Responsibilities: Perform comprehensive resident assessments and document findings in accordance with facility policies and regulatory standards. Develop, implement, and evaluate individualized care plans based on the resident's physical, emotional, and social needs. Administer medications, treatments, and procedures safely and accurately as ordered by the physician. Monitor changes in residents’ health status and respond appropriately to emergent and routine care needs. Collaborate with interdisciplinary team members—including CNAs, LPNs, therapists, and physicians—to coordinate continuity of care.
