Salary$22.84 - $39.74 Hourly$47,510 - $82,667 AnnuallyThis position is a Pay Band 80Purpose of PositionTHIS POSTING WILL BE USED TO CONDUCT ONGOING RECRUITMENT AND WILL REMAIN OPEN UNTIL POSITION HAS BEEN FILLED.Why does the job exist?The purpose of the Registered Nurse-Advanced Families FIRST role is to provide comprehensive care coordination to Medicaid-eligible pregnant women and children up to age three. The care coordinator is directly involved in the assessment and evaluation of pregnant women and children ages 0-3. The Families FIRST RN-A ensures the management, coordination, and monitoring of target population in areas of medical, social and educational needs to improve birth outcomes of pregnant women and the health status of their infants as outlined in the Families FIRST model. The Families FIRST RN-A establishes a referral network and collaborates with community groups to educate clients on the importance of early prenatal care and preventive services for pregnant women and their infant children. The Families FIRST RN-A provides education to clients on pregnancy and childhood development and refers clients to community resources to assist them in achieving full potential for their families.How does it get done?The RN-A Families FIRST nurse must utilize effective communication among multidisciplinary team members, providers, and physicians in reference to optimal patient care. The RN-A Families FIRST nurse must be able to access and refer clients to other community programs based on their professional nursing assessment, and be able to act quickly if a situation calls for immediate intervention. The RN-A Families FIRST nurse must make program-specific decisions per protocols, and prioritize client needs accordingly. This position requires administering nursing care to clients by utilizing the laws, principles, practices and techniques of professional nursing, as well as a working knowledge of billing and Electronic Health Record data entry. The RN-A advises on health maintenance, disease prevention as needed in the areas of Family Planning, STDs, Immunizations and Infectious Disease per Protocol utilizing the CACTUS data base as appropriate while maintaining HIPAA and complying with procedures outlined in the Families FIRST manual.Who are the customers?Medicaid-eligible pregnant women and children ages 0-3.Ideal CandidateThe ideal candidate will have experience with pregnant women in an OB clinic or hospital setting such as Labor and Delivery. The ideal candidate will have experience with infants, toddlers and children, ages 0-3 yrs. Experience in case management and bilingual (Spanish) is preferred. The ideal candidate will be a self-starter and able to work independently as well as with a team.Minimum QualificationAssociates Degree in Nursing or graduation from a NLN accredited program accepted by the New Mexico Board of Nursing and three (3) years paid experience as a Registered Nurse. Must be licensed in accordance with the Nursing Practice Act -- 61-3-2 through 61-3-31 NMSA 1978 and 16.12.1 through 16.12.20 NMAC, as applicable.Substitution TableNo substitutions. Must possess required licensure, certification or registration and required number of years of experienceEmployment RequirementsMust possess and maintain a valid Driver's License. Pre-employment background investigation is required and isconditional pending results. Must be licensed in accordance with the Nursing Practice Act -- 61-3-2 through 61-3-31 NMSA 1978 and 16.12.1 through 16.12.20 NMAC, as applicable. Must maintain current CPR certification.Working ConditionsWork is performed in an office setting with exposure to Visual/Video Display Terminal (VDT) and extensive personal computer and phone usage.
